If you have returned an item but it still appears on your account, use this form to claim it to avoid fines.

• We will search for the returned item for 30 days
• If the item is not found, you may be charged with replacing it
• You will receive an email if the item is found or if the search has ended
• You have the option of sending a copy of your claim to yourself after submitting
